Use this checklist to organize recent observations, current medicines and supplements, documents, and the questions you want to ask at a child’s appointment. It is a preparation aid, not a medical assessment.

- Write the dates and times of the observations you want to discuss.
- Note sleep times and night wakes when relevant to your questions.
- List meals, ingredients, activities, bowel observations, or behavior records you plan to bring up.
- Mark missing days as unrecorded rather than guessing.

- List supplements with name, dose, and unit.
- Bring pharmacy labels or photos if that is easier.
- Write down allergies and sensitivities already known to the care team.
Documents to bring

- School or therapist notes you already have permission to share.
- Your short timeline or printed observation sheets.
Practical details

Daily logging, child profiles, routine plans, reminders, the timeline, basic comparisons, and single-event reviews are free.
