Use this sheet to write down three parts of one observed event: what was happening immediately before, the behavior you observed, and what happened immediately afterward. Keep entries factual and time-specific.

ABC observation sheet
ABC is a simple structure for organizing observations: A — antecedent or setting, B — observed behavior, and C — consequence or what happened next. The labels organize a record; they do not establish why an event occurred.

Recording tips
Write what another person could observe
Use concrete details
Include the time, place, people present, exact words when relevant, and the visible sequence of events.
Separate facts from interpretation
“Covered ears for 20 seconds” is more specific than a broad label. Leave unknown details blank rather than filling them from memory.
Record positive moments too
The same structure can document calm transitions, requests for help, and activities that went as planned.
Keep one event per row
A single sequence is easier to read later than several events combined into one entry.
